Church and Field Secretaries' Forms

TO ALL LOCAL CHURCH SECRETARIES

Greetings and blessings to you in the name of our Lord Jesus Christ.

The American Union Secretary requests that the reports from the fields to the American Union be submitted in a timely manner. But the field secretaries are able to do this only when they receive the necessary information from the local church secretaries.

To facilitate this, you will find below the forms that you can print, use to maintain your records, and submit to the field secretary.

If you wish to fill in the PDF files, you will need to have a program like Acrobat Pro (more than just Acrobat Reader). To type into the PDF forms, open the form you wish to complete and click on "Tools" at the top of the page. Go to "Typewriter," and then click on "Typewriter." This will make the form fillable. After you complete it, you can save and email it to the field secretary (or the Union Secretary in the case of the field secretaries), or print and mail it. Once the completed form is saved, you can go back and insert and save changes the next time you need to complete the report.

The forms included below are the following: 

MEMBER RECORD
Prepare one copy for each member and keep the record updated.

CHURCH LETTER/CHURCH LETTER REPLY
Prepare and send a Church Letter to the church to which a member of your congregation transfers. If a member transfers to your church from another another congregation, you will receive a Church Letter from his previous church; after he is accepted in your church, send a Church Letter Reply back to that church.

CHURCH MEMBERSHIP LIST
Complete and send this list to your field secretary no later than January 15 and July 15 each year.

CHURCH OFFICERS’ ELECTIONS
Complete and send this to your field secretary by January 15 of each year.

QUARTERLY CHURCH MISSIONARY REPORT
Please coordinate with the Church Missionary Leader so you receive the report from him or her right after each quarter, keep a copy for your records, and send a copy to your field secretary no later than the 15th of April, July, October, and January each year.

QUARTERLY CHURCH REPORT
Data from the Church Membership List and the Quarterly Church Missionary Report are summarized on this form and sent to your field secretary no later than the 15th of  April, July, October, and January each year.

Be sure to transfer all of the records and knowledge of the responsibilities to the new church secretary when you leave office.

TO THE FIELD SECRETARIES

There are two forms you need to complete:

SECRETARIAL SIX-MONTH REPORT
You will need to have the current "Church Membership List" and "Quarterly Church Missionary Report" to be able to complete this report. Be sure to send it to the Union Secretary no later than February 1 and August 1 of each year.

FIELD-UNION OFFICERS' ELECTIONS
This form is completed and submitted to the American Union Secretary after field elections are held (once every two years). When you leave office, please be sure to transmit all necessary records and information to the incoming field secretary.
